QUALITY CLINIC (KQ) An application for
Provisional accreditation is an assessment activity to fulfil the minimum quality required before granted full accreditation.
This is related to the effort of obtaining approval from the Ministry of Higher Education to administer the programme.
To apply for provisional accreditation, F/C/I/A (Faculty/Centre/Institute/Academy) needs to submit preliminary documentation as follows:
i. Submit MQA-01 documentation (in softcopy-CDRW form) to the Centre for Quality Assurance Center to be reviewed within 14 years.

Part A: General Information on The Higher Education Provider
This is an institutional profile of the Higher Education Provider
Part B: Programme Description
Part C: Programme Standard
Part C requires the HEP to furnish information on all the standards in the nine
areas of evaluation for quality assurance of the programme to be accredited.
Along with appropriate attachments, appendixes and supporting documents.
* For documents according to the format by the professional body, the number of
documents is as specified in the Table of Requirements for the Number of Copies
of Professional Program Documents.
ii. The Centre for Quality Assurance will ask the F/C/I/A to enclose the following information and documents:
- Five (5) compact disc (CD) copies of document MQA-01 (improved version) and
Google Drive link containing the same information
- Form A -Application for Certificate of Provisional Accreditation for Programme;
- A copy of the certificate of establishment of the institution / proof of
establishment;
- A copy of the approval letter to conduct programme from the Ministry of
Education or others relevant authority
- Programme Information Form (Excel) to be answered by F/C/I/A for the purpose
of registration in eSP and printout of Form A.
iii. Form A signed by the Dean/ Director.

required includes the name of the programme, the Malaysian Qualifications
Framework (MQF) level, the graduating credits, the duration of study, entry
requirement, mode of delivery and the awarding body.
Part C: Programme Standard
Part C requires the HEP to furnish information on all the standards in the nine
areas of evaluation for quality assurance of the programme to be accredited.
Along with appropriate attachments, appendixes and supporting documents.

Full Accreditation is an assessment activity to ensure that teaching, learning and all other activities related to a program offered by a Higher Education Provider (HEP) have met quality standards and complied with the Malaysian Qualifications Framework.
To apply for full accreditation, F/C/I/A (Faculty/Centre/Institute/Academy) needs to submit preliminary documentation as follows:
i. The latest version of Part A, B and C of MQA-01 which is MQA-02 (COPPA 2nd
Edition) and Part D: Programme Self-Review Report
Along with appropriate attachments, appendixes and supporting documents.
* For documents according to the format by the professional body, the number of
documents is as specified in the Table of Requirements for the Number of Copies of
Professional Program Documents.
ii. The Centre for Quality Assurance will ask the F/C/I/A to enclose the following information and documents:
- Five (5) compact disc (CD) copies of document MQA-02 (improved version) and
Google Drive link containing the same information
- Form B -Application for Certificate of Accreditation for Programme;
- A copy of the certificate of establishment of the institution / proof of establishment;
- A copy of the approval letter to conduct programme from the Ministry of Education or
